On your initial visit, you will go through a physical examination and discuss your condition, medical history, and lifestyle, as well as any questions or concerns you have. You will likely undergo some diagnostic tests, and depending on the results, talk about the appropriate treatment options, the benefits and risks of each, and the plan moving forward.

What should I bring to my appointment?

Plan on bringing your medical records, copies of imaging tests if you have them, your driver’s license or picture ID and your insurance card. If applicable, bring along your referring physician’s information, too, as well as a list of your current medications and any questions you may have. You can also accelerate the check-in process by downloading and completing any patient forms ahead of time and bringing them to your appointment. If you have any questions at all, just call and our friendly staff will be happy to assist you.
